The assesment is the most important part of nursing process. The assessment of the patient is done on priority basis I.e from most important to less important condition. During assesment, life threatening conditions are treated first.

Let us understand by an example

If the patient is suffering from hypoxia (less oxygen in tissues) the patients tissue became dead due to less supply of oxygen to tissue.

In hypoxia the immediate response of the nurse is to determine the exact cause of the hypoxia and respond quickly.

As per the ABC(airway,breathing,circulation) rule the next and immediate action of the nurse is to prevent hypoxia by maintaining the airway and by providing oxygen to met the oxygen demand of tissues.
