During the shift report, the nurse reported that she pulverized Ms. Dee’s medication and administered them with applesauce. However, Ms. Dee still had difficulty swallowing the medication. You will be responsible for reporting Ms. Dee’s health care needs at the interprofessional team conference.

(A) I would report as Ms. Dee has been constantly finding it difficult to swallow her tablets. While performing alternative ways, thinking that it can help was in vain. Tablets were pulverized and mixed with apple sauce. It was still difficult for her to swallow. I would prefer parenteral way of administering the tablets to the patient.
