(A) I would report as Ms. Dee has been constantly finding it difficult to swallow her tablets. While performing alternative ways, thinking that it can help was in vain. Tablets were pulverized and mixed with apple sauce. It was still difficult for her to swallow. I would prefer parenteral way of administering the tablets to the patient.
