Nursing DIAGNOSIS for pre eclampsia -

  • An elevated BP of more than 140/90 mm hg after 20 weeks of pregnancy.
  • Presence of protein in urine .
  • Patient may have edema in feet and face.
  • Ineffective tissue perfusion leading to vasoconstriction of blood vessels.

PLAN AND EVALUATION-

  • Maternal evaluation includes monitoring BP, urine output signs and symptoms like headache , visual disturbance and abdominal pain.
  • Ask the mother to take adequate rest.
  • Monitor blood pressure regularly.
  • Monitor urine input and output frequently in a day.
  • Administering ANTI- hypertensive on physicians advice.
  • Look for any signs of fluid overload.
  • Check for fetal heart rate .
  • Provide Nutrition and lifestyle education.
