In a health care facility, such as a hospital, nursing home, or assisted living, an incident report or accident report is a form that is filled for recording the details of any unusual event such as an injury to the patient that occurs at the facility. An incident form is an administrative job generally not part of the health record. If the incident report is mentioned in the chart, then it becomes part of the health record. Since it is part of administrative process it is not part of health record.
