Option d . Raynanud syndrome is nothing associated with vasculitis. It is cold or numb response to stress or very low temperatures and seen mostly in hand , fingers, toes.

Now, option A,B,C these all are associated with vasculitis ( blood vessels inflammation) and these have no exact cause of happening these diseases( that may be viral or bacterial or may be genetical ). From these polyarteritis nodosa and wegner's granulomatosis can occurs in any age of life ( children to old age ).But KAWASAKI DISEASE is exclusively occurs in children ( mostly affects children less than 5 years) as similar clinical history presenting in this question. So as the patient is a child for the nurse should firstly prioritize Kawasaki disease.
