2Inspect stoma and peristomal skin area during the changing of pouch.check for irritation ,bruises and rashes.Rationale-Helps in monitoring healing process and effectiveness of appliances and identifies areas of concern,need for further evaluation and intervention.Early identification of stomal necrosis or fungal infection which helps in timely prevention.
