Documentation is the most important part of nursing care. It is the last and evaluatory part of nursing care in which we are documenting or recording the patient condition, response to nursing interventions and nursing care that we are provided.

Documentation is important because of it provide appropriate and liegal document for patient care. If any medico liegal cases occur documened record or patient file act as an liegal document. And also it will helpful in identifing the past medical history as well as there is any allegies or contraindications to any procedure can be check.

Nursing care is not only given by oneself but also there are many nurses involved in caring a patient and also other medical professionals are also involved in the healthcare. So proper documentation after the implementation of procedure is needed to avoid the repitation of procedure.
