At the airport you overhear a woman say, "I give my child a dose of Benadryl before a flight so that he will sleep." You know that this is not the intended use for Benadryl. Being a nurse in the public transportation area like airport the nurse can introduce herself and check whether the mother is giving this medication after consulting with the pediatrician or not.

Ethically pediatrician don't order for this option unless the journey is a very long travel explaining the side effects also.

She should have tried this in home as well and assessed for any side effects because some children gets hyper after this drug administration.

Legal implications like if the baby facing any complications of the drug during flying the mother can be sued if there is no pediatrician consultation.The nurse can face problem if she is aware of it and it forms a negligence on her part if she avoids informing it to the superior.
