1) Finasteride
Generic or brand name - Proscar and Propecia
Category class -5 alpha reductase inhibitor
Expected pharmacological action -by inhibiting 5-alpha reductase ,thus preventing DTH production, finasteride reduces androgen signalling in tissues like prostrate and scalp.
Complications-decreased sex drive, ejaculation disorder, skin rashes, increase in breast size and tenderness.
Contraindications or precautions-breast cancer in male patients, pregnancy, liver problem, high grade prostate cancer, suicidal thoughts.
Interactions-interact with low dose aspirin, crestor, fish oil, minoxidil.
Medical administration-by mouth
Evaluation of medical effectiveness-finasteride is evaluated as effective by significant decrease in TPV by 40% even after 12 months.
Therapeutic use-to treat hair loss in men and enlarged prostate gland. It is also used in women to treat excessive hair growth.
Nursing interventions -evaluate carefully whether there is a increase in serum PSA level while patients taking finasteride. It may indicate presence of prostate cancer or non compliance with the therapy.
2.Tamsulosin

Category class-alpha blockers
Expected pharmacological action-it is alpha-1 adrenergic blocking agent exhibiting selectivity for alpha 1 receptors in human prostate. Relaxation smooth muscles in the bladder neck and prostate produced by alpha 1 adrenergic blocker results in improvement in urine flow rate and reduction in symptoms of BPH.
Complications-dizziness, lightheadedness, running nose, drowsiness, ejaculation problem.
Contraindications or precautions -sulfonamide hypersensitivity, prostate cancer, hepatic disease, pregnancy, renal impairment.
Interactions-they interact with cimentidine, antibiotic, antidepressants, antifungal drug, benign prostatic hyperplasia drug, alpha blockers.
Medical administration -by mouth
Evaluation of medical effectiveness-vital sign changes in hypertensive patients were are not clinically significant. Tamsulosin once daily has shown effective, safe in BHP population.
Therapeutic use -treat enlarged prostate that is benign prostatic enlargement.
Nursing interventions -make position changes slowly to minimise orthostatic hypotension. Report dizziness,vertigo and fainting to physician. Exercise cautionn with hazardous activities.
