You are an APN in a Quick Care Clinic. Mom brings Joey, her eight-year-old son, in for possible GAS pharyngitis. You notice that Joey has been seen and treated multiple times this year for the same condition. Your clinic does not have the ability to culture, and mom refuses the rapid test “because it is always negative, but I know he has it.”
According to the article by the Infectious Disease Society of America, how would you treat Joey? Be sure to support your answer. Could he be a chronic pharyngeal carrier?
A chronic pharyngeal carrier is one who has this strep bacteria as carrier and may or may not show signs and symptoms of infection, as this child is having multiple times of visits .The line of treatment is antibiotics (penicillin group) to compete the entire course .In case of a carrier ,the patient's doesn't exhibit any signs and symptoms but here the child is brought with complaints of GAS Pharyngitis.
Doing antibody screening test at this time may give a f as lose negative report ,so the child has to under a serum antibody test for strep after 6 weeks to get it confirmed. The gold standard diagnostic test is a culture of the throat swabs. The child can be suggested for a tonsillectomy because this can prevent the recurrent infection for next 2 to 3 years.
