1. Menstruation history

- First day of Last menstruation date ( to calculate the EDD)

2. Gynecological history

- history of reproductive surgery

- history of any genital herpes, infertility, uterine abnormalities, STI, PID, breast surgery, breast disorder

3. Obstetric history

- Number od gravida

- Number of parity

- previous history of previous delivery and types of delivery

- gestational age

4. Current pregnancy

- History of any symptoms during bleeding, abdominal, discharges, etc.

- fetal movement

- folic acid intake

5. Medical history

- Any allergy to the medication

- History of intake of alcohol, smoking, drug use, tobacco, anemia

6. Family history
