EHRs contain types of patient data are adminstrative and billing data, patient demographics, progress notes, vital signs,madical histories, diagnoses, medications, immunization dates, allergies, radioiogical images, lab and test results.sources of data in EHRs are electronic medication adminstration records and laboratory information systems, radiology inforamtion system, hospital inforamtion system, nursing informationsystem etc.and other data sources are primary and secondary sources, the primary sources are incorporate data collected for direct purpose of the registry and secondary data sources are comprised of data originally collected for purposes other then the registry under consideration(e.g, standard medical care , insurances,claim processing), by this it shows that the primary source of data collection increases the probabilityof completeness, validity, and reliability.
