The process of care in nursing involves assessment which includes collecting only the physiological data, but also information about the psychological, sociocultural, spiritual, economic, and lifestyle factors as well followed by the nursing diagnosis. Based on the assessment and diagnosis, the nurse sets measurable and achievable short- and long-range goals for a patient that includes many things such as; moving from bed to chair at least three times per day; maintaining adequate nutrition by eating smaller, more frequent meals; resolving conflict through counseling, or managing pain through adequate medication. This is followed by the implementation of the care plan, its documentation and evaluation in the end.
