It is essential to asses the patient understanding about the in home medications post discharge.

The nurse can assess the patient using medication assessment protocol inorder to evaluate patients ability to have medications on time.

Instructions include: Asking the patient to demonstrate the medication preparation and intake of medicine- it helps the nurse to assess wether the patient needs assistance or he can do it independently.

Ask the patient to describe the proceedings like which has to be taken before and after food- It helps to assess the patients knowledge on dosage, frquency and time of drug.

Check adherence to drug by diary, review calendar and by counting the pills from start date to till date etc.
