Acute pain related to destruction of skin tissue as evidenced by facial grimace, report of pain, restless,pain scale

Nursing interventions and rational:

Provide bed cradle to maintain temperature

Cover wound to prevent nerve ending exposure to environment to minimise pain

Elevate the burnt area to prevent edema

Position frequently to relieve pressure

Assess the level of pain

Diversional therapy

Administer medications like analgesic as per order

Desired outcome:

To control pain

Patient is diverted to other things

Gets adequate sleep and rest

Relaxed facial expressions
