#. Poor EHR system design and improper use can cause EHR-related errors that can effect the integrity of the information in the EHR, leading to errors that endanger patient safety or decrease the quality of care.

These unintended consequences also may increase fraud and abuse and can have serious legal implications.

Since EHRs involve a digital format, there are concerns of breach. Internet hackers do have the potential to access confidential medical information.

The conversion from a paper based system to EHRs leaves the risk of misplacing data throughout the process.
