The
patient Abdulla is a 54-year-old man who has made a living as a
construction worker. He left high school at age 16 and has been a
laborer ever since. He never saw any need for "book learning," and
has lived his life "my way since he was a teenager. He has smoked
one pack of cigarettes every day for 40 years and follows no
special diet, eating a lot of "fast food" while on the job, Abdulla
admitted yesterday to the coronary care unit for complaints of
chest pain and he is scheduled for cardiac catheterization in the
morning. The patient has not signed the informed consent form for
the procedure. The patient is restless and worried about the
procedure and the result, as it will affect his job and income. He
is walking up and down the room quickly, with his hand on his chin,

daughters. His wife is very anxious and keeps crying in between.
His daughters are in high school and soon will start their higher
studies. The wife keeps telling him in between, "I told you several
times, you should have stopped smoking, see what happened now?" He

to support my kids if I have a bad result." The nurse Amna walks
into the room quickly and asks the patient, "Why have you not
