Nursing process is a scientific method consist of a series of steps by which a nurse assess patient's problems, plan the intervention and evaluating the actions after implementation of the same.

Nursing Diagnosis: Mr. Matthew ( patient)

1. Severe anxiety related to situational crisis   evidenced by disorientation, feelings of discomfort and disorganised thought process/ personal identity disturbances.

2. Ineffective individual copying related to situational crisis ( loss of job and loss of partner), evidenced by inadequate problem solving and inability to meet basic needs.

3. Personal identity disturbances :

Disturbed personal identity related to severe level of anxiety evidenced by unable to identify himself and disorientation .
