Yes it took almost 50 years

But it's not such a long period of time , the main reasons are

  1. Slow progress in the development of health care sector is the main reason for reason for the slow progress in area wide planning
  2. Lack of planning and execution- many countries did not gave importance to the development of health care sector so it brought about a pull back
  3. The importance of locally arising disease and the statistics about these conditions were released very slowly. This statistics about locally arising disease and their importance was understood largely
  4. Realisation that when local problem are dealt locally then cost of care can be reduced. This was found lately
  5. Enlightenment on the importance of local level hospital and establishments for area wise division came into notice very lately

These are the main reason that cause slowing progression
