Patient should be aware of the treatment plans and physician should speak to and listen to patient.Physician is accountable for his or her actions to the patient. For example, physician takes decision( surgical intervention)without communicating with the patient.
Dereliction: Dereliction is the failure to meet or serve the patient in an expected standard of proffession. For example: administering wrong medication.
Damage: The adverse reactions due to any kind of negligence. It can be serious or fatal damage. For instance, wrong patient underwent wrong surgery leads to organ loss ( wrong part amputation).
Direct cause: The negligence lead to direct negative effect on patient . For example, wrong medication caused serious adverse reactions on patient.
