Misfiring the wrong patient name can happen very easily.

One reason can be a common software practice problem. When scanned documents are to be assigned to a patient's name then the name is selected from the drop down menu from a the patient's name. Whole this can be correct for almost every time but when there is a pressure of work completion anyone can make this mistake and choose wrong patient's name.

Another reason can be when there is no other data then the patient's name like no birth date then when there are patients having similar name will lead to a mistake by a person who's feeding the data by making an educated guess
