Often she feels some cloudy appearances while looking at objects and has watery eyes. Also she noted that her knees are swollen and very painful. Her doctors prescribed analgesics for to control pain. However, when her children visits, she often grumbles about her pain and the children takes her to different doctors and sometimes get medication from over counter too. All these medications leads to defecate difficulty. You are the assigned nurse to visit her. Your home visit history collection reveals that she has Insomnia and sleeps only 4 hours in the night because of her knee pain. She had complaints of anorexia and always avoid eating her breakfast and totally she eats only one third of her daily requirements. Has mild cough and during each cough she has dribbling of urine. Her bed sheets needs to be changed at least once in 6 hours. Also you observed that her son and Mr. Said is trying to position her straight in bed but she was sliding down. Ms. Yammah has limited sensation to feel that she need to be turned or pulled up. Her son said that for past 4 months he is taking her in wheel chair to the bathroom for to meet her hygiene needs. The following drugs Ms. Yammah is on: Tab. Amitriptyline 10 mg Hsod; Atrovastatin 40 mg OD; Tab. Acetamenophen 1 gram sos; Tab. Atenalol 50 mg OD; Syr. Multi vitamin 20 ml BD.Syrub. Lactulose 20 ml TiD. You are educating the family based on the assessment before you complete the home visits.

Some measures that reduce patients pain are

- massage and positioning :- explain patient and son about mild massages can help to reduce the pain, and also advised patient's son about proper positioning with adequate comfort measures and repositioning after every 2hours

-explain about safe environment, with some modifications such as reduce noise and light can help to reduce the pain and the patient can sleep

-hot compress can be applied to reduce the pain and swelling.

-music therapy also will help to reduce the pain and can be manage insomnia.
