can you assist me on how to write a memo to my administrator on a problem of how paper medication administration has become obsolete due to numerous medication errors and a solution of how implementing a computerized electronic medication system will help alleviate the problem

  • Paper medication has become obsolete due to numerous medication error during prescribing. Wrong medication , wrong dose, wrong frequency are mostly done by the physician. It accounts 50%of medication error. One of the major cause is distraction due to heavy duties in hospital to the physician . Other is distortion due to bad writing , not able to understand the symbols and abbreviations. Illegible writing also lead to such error. To overcome such error, use of electronic records are done where everything is typed and properly monitored.
