Scenario 1: Target population: African American women, health issue: heart disease, participants have participated in a healthy eating cooking class for 6 weeks.

Scenario 2: Target population: elementary school children, health issue: obesity, participants have participated in a after school P.E. program for the entire school year

What survey questions (5-10 minimum) would you ask this target population as it relates to the program they have participated in?
