One memorable encounter during an emergency transport, quickly escalated into an emergency intubation inside the pediatric ICU. I arrived on the scene of an unresponsive elderly patient in a nursing facility who was experiencing respiratory distress, diaphoresis, with an oxygen saturation below 93%. My partner and I proceeded to administer 10 liters of oxygen to stabilize him. Soon after the patient was loaded onto the ambulance, the patient began to vomit. To maintain patency of his airway and prevent aspiration, it required me to perform liberal suction. Brookdale Hospital is a level 1 trauma center, and the line to be triaged at the emergency department went around the block during the height of the pandemic. The task alone of moving our patient on the stretcher and onto the line was overwhelming. I needed to remain focused on my patient who was perhaps a brother, a dad, a grandfather, and a husband to his loved ones. Suddenly we were met with a triage team which included a nurse, PA and an attending. They explained the nature of the virus, diverted certain patients in contact isolation, and how they did not have enough beds for all patients. Our patient on the stretcher was quickly assessed by the PA who seemed to take charge, and informed me the need to be intubated. This practice was outside of my training, so we had to work together, as the PA along with his supervising physician performed the procedure on our stretcher in the pediatric ICU. The adult ICU was occupied with COVID-19 patients. During the procedure, I cut the clothes from my patient as the PA stabilized the patient. He used an intraosseous drill to insert a needle and port into the hip. The physician began feeding the newly inserted line and taking blood samples as I watched and yearned help more. I was told to differentiate between blood vials needed for testing as the ED team worked together in perfect harmony focusing on my patient. It was this precise moment when I realized I wanted to learn more, and the patient-care oriented PA was the perfect profession for me.
