One innovative health care delivery model that incorporates an interdisciplinary care delivery team is; Patient-Centered Medical Home. A team-based model housed in primary care “the Patient-Centered Medical Home (PCMH)” aims at delivering a coordinated care over time. Its tools include the electronic health records, disease registries and practice guidelines, and continuous quality improvement initiatives that may be used by the PCMH for tracking the patient referrals and treatments, including those supplied by specialist physicians. Based on the evidence it can be suggested that the quality of care, patient satisfaction, and access to care have improved by PCMH models. The advantageous patient outcomes for this include; access to health records remotely leading to improved healthcare at home.
