diagnosis- nausea related to effect of hepatitis b,as evidenced by patient complaining of nausea

goal- reduce nausea

planning- the patient will report decrease nausea with in 6 hours the patient will tolarate liquits with in 12 hours

intervention the nurse will administersed the inj. zofer 4mg iv every 6 hrs ,the nurse will assess for nausea every 2-3 hrs

evaluation patient nausea reduced,no nausea now
