Identified nursing practice issue for readmission related to the topic I have chosen:

My topic: Preventing 30-day hospital readmissions

Issue: The practice issue I have decided to focus on deals with psychiatric patients and the frequency of readmission back into the hospital within 90 days of being discharged. So, my PICOT question is: In patients discharged with psychotropic medication (P), how does a 24 hour follow up phone call with patients (I), compared to giving patients standard discharge instructions (C) decrease hospital readmissions due to noncompliance with medications (O) for 90 days (T).
