Dr.XXX ,this is nurse Y calling from Geriatric ward regarding the patient Mr.W admitted by some gastrointestinal problems and needs your advice to sort the current issues

Background

Mr.W was administer with known treatment for neurological disorder and now is complaining if altered bowel pattern.The patient cannot control the elimination pattern.

Assessment

The patient has difficulty in formation of stool .It seems to bre the patient has semi formed and sometimes non formed stool to eliminate

Recommendation

I request the stir ybcan be provided some fiber supplements to ensure proper stool formation and regulate his elimination pattern
