If the first attempt to insert a cannula for the IV was unsuccessful, what nursing measures should the nurse take before another attempt is made?
The IV has been started and is infusing by gravity drip. What teaching considerations should the nurse use to instruct this patient?
Answer: Infiltration occurs when I.V. fluid or medications leak into the surrounding tissue. Infiltration can be caused by improper placement or dislodgment of the catheter. Patient movement can cause the catheter to slip out or through the blood vessel lumen. Assess the IV insertion site and transparent dressing on IV site. Check IV insertion site for signs and symptoms of phlebitis or infection. Check for fluid leaking, redness, pain, tenderness, and swelling. IV site should be free from pain, tenderness, redness, or swelling.
The patient is demonstrating signs of infiltration or extravasation. The nurse should stop the infusion, discontinue the cannula, slightly elevate the extremity, and notify the RN. Cool compresses are used for infiltrated IV fluids.
