Answer: When the pulse generator is inserted at the same encounter, report 33202 or 33203 in addition to:

  • 33212, insertion of pacemaker pulse generator only; with existing single lead
  • 33213, insertion of pacemaker pulse generator only; with existing dual leads
  • 33221, insertion of pacemaker pulse generator only; with existing multiple leads

If, during the insertion process, the skin pocket requires revision, this is also included in these codes (and all others from 33206-33249):

  • 33206, insertion of new or replacement of permanent pacemaker with transvenous electrode(s); atrial
  • 33207, insertion of new or replacement of permanent pacemaker with transvenous electrode(s); ventricular
  • 33208, insertion of new or replacement of permanent pacemaker with transvenous electrode(s); atrial and ventricular
