External Cause Codes are most often reported secondarily to codes from nearby chapter 19, Injury, poisoning, and certain other consequences of external causes (S00-T88). Chapter 19 codes begin with the letters S or T, and this is where codes for acute injuries are found, such as those sustained in an automobile accident.
If a physician were to select a code such as S13.4xxA Sprain of ligaments of the cervical spine, initial encounter, it’s also appropriate to report the external cause of the injury. The S code would act as the primary diagnosis; external cause codes can NEVER be reported first.
Here's the case:
A 12-year-old boy presents with his father to the ER due to open wounds in his arm, hand, and upper leg. The injury occurred when the boy fell on a barbed wire fence at the farm while running in the field. Diagnosis: Multiple lacerations to the right forearm, right hand, and left thigh.
