As, the line should cut because if it not cut then the start mark can be overlooked and the wrong medicine may be administered by other nurse. And proper brief explanation has to be given to prevent any confusion and proper communication to other health personals. The signature of self and witness is required to prof that is is rechaked by another stuff.
