Post acute care-- Followed by acute illness covering of rehabilitation and home care services including palliative and hospice care.

Acute care payment - Fixed payment based on diagnosis .

Providing fixed payment based on diagnosis creates an inherent financial incentive for acute care hospital to underserve facility to the most severely impaired patient.

Acute care hospitals has to cover the post acute care service also. it means super market approach covering of ICU medical and surgical followed by rehabilitation and palliative care.

Post acute care will be different for each patient condition . So acute care administration or administrator difficulty to negotiate all facilities with in that fixed payment
