The third heart sound is S3 or ventricular gallop, It starts at early diastole.

The fourth heart sound is S4 or Arterial gallop, it starts just before S1.

S3 and S4 occur in due to Cariomyoapathy or Herat failure which resulted in Harding of Ventricle, forcefull blood flow to ventricle from atria due to poor relaxation and other various abnormalities.
