History collection is one of the important tool for disease diagnosis . A proper history reveals basic datas about the client condition . History collection includes datas about age ,sex , occupation ,edication ,marrietal status , economic datas , family history , previous history of medical and surgical history , communicable diseases , allergic conditions etc .. Major illness like cardiac diseases and DM , cancer , congenietal anomalies etc are realated to family history .. n Before taking prper history the NU should maintain a good interpersonal  relationship with the client and explain about the importance of history collection
