The clinical manifestation should typically include for this patient who is an elderly woman with hip fracture should be dementia with the complaint of; severe hip pain, with the inability to walk.

A nursing diagnosis would be hip fracture due to severe osteoporosis.

The most important outcome measures management of the pain, regaining of mobility and regaining of independence. I choose this diagnosis based on the fact that she is a woman and that to of age, so it is likely that she is going to suffer from osteoporosis after menopause.
