SOAP stands for subjective, objective, assessment and plan.

This is used for documentation purpose, for patient disease history, physical examination, identification, sign and symptoms etc

History of present illness includes patient identification, history of sign and symptoms of disease that patient experienced. All this information comes under subjective data in which health care professional ask question regarding patient condition.
