Ordered vancomycin is 40mg/kg/day every 8 hours

Weight of child in kg is 13.5 kg if 1lb is equal to 0.45 kg then 30lbs in kg will be 13.5kg

To calculate-- drug dose per day for 13.5kg child

If in 1kg drug dose is 40mg/day

So, x mg /13.5 kg= 40mg/1kg

x= 40mg /1kg × 13.5kg = 540mg

Or by DA , amount of drug that client will receive per day

0.45kg/1lb × 30lbs × 40mg/1kg = 540mg

Thus the total amount of drug that the client will receive per day is 540mg

( Question is done by DA method that is dimension analysis which is used in pharmacy calculation )
