Dose in hand is = 50 mg tablet

Ordered dose / dose in hand =5/50= 0.1

It is difficult to take so it can be dilute it in 10 ml of  water  

So 5/50×10=1 ml

Here the order is 5 mg po daily and not mention about related to patient weight also .as long as patient weight is not considering and also frequency not mentioned this dose may be not sufficient.

For children per oral medications can be given in the dissolved form or sup form ....here dissolved in water and can be given to the child.
