4) Family with a 6- year- old child who sustains an injury resulting in permanent paralysis from the waist down.

5) Elderly couple with the wife diagnosed with Alzheimer disease and who cannot be left alone. Husband still drives. Daughter lives in the community.

B. Incident Report

You are working in a medical office, and a patient falls while a medical assistant is helping the patient to step up to the examination table. The physician is called, assist the patient to get up, and finds a large bruise on the right hip but no other apparent injury. The physician orders an x- ray of the right hip, and instructs the patient to telephone if any problems arise later. The physician documents the examination and findings in the patient's medical record. It is office policy to fill out an incident report for any fall that occurs in the office. Make a list of all information that you would have to include on the incident report.
