In my opinion, using ethics of egoism patient 1 should get the kidney as he has helped other people and also has a lesser time as compared to others. After the operation, patient 1 should help in finding the kidneys for the other two patients. As all the patients need the kidney but there is only one kidney available, rest of the two might die in the absence of a kidney. This is utilitarianism to a key moral conflict involving health care in this case.
