The following is an account of an emergency department visit for patient Sam Shady on July 5, 2000.

            On the morning of July 5, 2000, Sam Shady tripped over a crack in the sidewalk, injuring his left ankle. He hobbled back to his house and summoned his wife, Sarah, who drove him to the local hospital emergency department.

            On arrival at City Hospital’s emergency department, Sam was interviewed by the registration clerk. He was asked to provide personal information and health insurance information. He completed some forms for insurance payment and signed the consent for care form. Next, he was seen by the triage nurse. The nurse took his pulse, temperature, respiratory rate, and blood pressure. She asked about the details of the accident and examined Sam’s ankle. She wrote some notes in the medical record and told Sam to wait for the physician, who would be there in a few minutes.

            The physician arrived and examined Sam’s ankle. He ordered an x-ray film and told Sam he would be back to discuss the results. Sam was taken to the radiology department, where his ankle was x-rayed by the radiological technologist. The radiologist read the x-ray film and reported the results to the emergency department physician.

            The physician informed Sam that his ankle was not broken but was severely sprained. He was instructed on how to care for the sprained ankle and was advised to take acetaminophen (Tylenol) for pain as needed. Sam was instructed to follow up with an orthopedic physician if needed. The emergency department physician ordered Sam to use crutches for a few days. The physical therapy department of the hospital was contacted. The physical therapist fitted Sam for crutches and provided instructions for their use. Sam was then discharged home from the emergency department.
