SBAR means situation, background , assessment and recommendation.

situation : A 76 year old female on wheelchair having diabetes, hypertension and dementia.

Back ground : The supporting background information is that she had a pressure injury stage 2 dependent ,she is on insulin and betablockers.Her pulse is 80,temperature 97,O2 96% and Bp 140/60.

Assessment : My assessment of the situation is that she is having stage 2 pressure injury because she is on a wheel chair for prolonged period, she may be having redness or blisters on skin

Recommendation : I recommended that position change every 2 hourly and wound care are  important to solve this problem.
