Concent is the approval taken by any the medical staff before starting any procedure but the Informed Concent is that in which the family member who is responsible to take decision for the patient will be educated about the risk factors and importance of certain test or procedure like surgery for the patient in detail to take their decision that they should go ahead or not in case patient is in-patient.

Concent form is just the record that the informed consent is delivered.

Role of nurse: how to inform and whole to inform about the condition of the patient is the major role of the nurse.

In case the medical information of the person which is confidential and communicated via email or fax then should be attached with the confidentiality clause.
