Answer - First and important intervention for blood transfusion reaction is to stop the transfusion and change the iv tube down to the iv site and iv line should open with normal saline.

Immediate blood transfusion should stop to prevent early complications of transfusion reaction and prevention from hemolytic reactions .Normal saline help to improve blood volume or provide emergency medicines or help to overcome risk.

Second important intervention include nurse should notify the physician or blood bank about blood transfusion reaction because he will take necessary actions or help in mange emergency conditions.
