BN is a 69 year old female patient with a history of DVT who comes to your ambulatory care clinic and the physician at the practice has prescribed edoxaban for BN. You notice as part of the review of her medical records that her most recent laboratory results indicate that her renal function shows a CrCl of 47 mL/min. What do you recommend____________
What if her CrCl decreases to 29 mL/min? What do you suggest at this point in time for BN?
Normal creatinine clearance for women is 88-128 mL/min , Lower than this value indicates a decline in renal function. Wtih reduction in renal function directly acting oral anticoagulants such as Edoxaban will accumulate within the body with reduced clearance within the body.
From the laboratory data her creatinine clearance is found to be 47 mL/min, low dose edoxaban is recommended.
If the creatinine clearance falls below 30 ml/min, Edoxaban is not recommended in such type of patients because of increased risk of bleeding
