George is a 75-year-old man who presents to the hospital with a cough and increased breathlessness; he has thick green sputum and is feeling feverish and has a loss of appetite. He has a history smoking and of moderate COPD and is on inhaled therapy.  On examination of his chest, he has coarse sounding crackles heard over the base of the right lung, which do not clear on coughing, and there is a dull percussion noted in the right lung base. The provider diagnosed George with community acquired pneumonia.
