Nursing diagnosis

1) Impaired Skin / Tissue Integrity : related to mechanical trauma of surgical removal of skin and subcutaneous tissues econdary to Cesarean section.

2) Acute Pain r/t surgical incision : Pain related to abdominal surgical incision (on abdomen with a surgical incision of 4inches) manifested by verbal report of  pain & guarding behavior.

Objectives

1) Achieve timely wound healing without any complications.

2) Mother is able to have regular urinary elimination with no bladder distention; there are no signs of urinary infection.

3) Mother is able to demonstrate bonding and attachment behaviors with the baby.
