IN this patient have difficulty swallowing pills so, give as per

1) if 5 mg give 9 tablets to get 45 mg per day , (due to swallowing difficulty it may be give as powder , grind form)

2) Otherwise , 10 mg *4 tablets and one 5 mg tablet. total 5 tablets per day and total required dose 45 mg.

3) and one more option is , 10 mg tablets*3 and 5 mg tablets*3 , each tablets is 3 to get 45 mg and total 6 tablets.
